A young family faced an overwhelming situation when their twins were born prematurely and required immediate transfer to a tertiary care hospital for specialized neonatal treatment. With multiple providers involved and decisions coming quickly, the KCS Case Management team stepped in to help coordinate next steps and support the family through the process.
As the twins stabilized, our nurse case manager worked with physicians and discharge planners to determine when a transition to a lower level of care was medically appropriate. Once it was safe to do so, KCS arranged for the infants to be moved to a local hospital, where they could continue inpatient neonatal care closer to the family’s home and without the high costs of a tertiary facility.
KCS Case Management’s coordination ensured the twins received appropriate care throughout their recovery while helping avoid unnecessary expenses.
